Textbook/Device Distribution for Fall 2020

Textbook and Electronic Device Distribution:

Step 1. Book your appointment.

Step 2. Review the District-required forms (click here to read forms).

Step 3. Complete the Google Form to indicate you have read and reviewed the District-required forms (click here).

Step 4. Print and complete the following four forms that you will bring to your appointment: Responsible Use Policy, Media Release (if applicable), Student Emergency Form, and Student Housing Questionnaire (click here to print forms) or download the attached files in English or Spanish. If you are borrowing a device or hot spot, complete the device form as well (attached below).

Step 5. Come to your appointment with your high school or middle school student ID. Wear a face cloth covering and adhere to social distancing requirements.

Instructions

Please use the following link to book your textbook/electronic device distribution appointment.  Distribution will take place in the Big Gym. You may park in the Vanalden Lot nearest to the swimming pool and walk in through the gate between A Hall and the Gym (there will be signs).    

Appointment dates are based on your grade and last name.  Makeups for all grades will take place August 18 (possibly on Saturday, August 15 as well – we will let you know).

If you need to fix your fall semester schedule prior to your appointment, please email your counselor before August 1 as we will base your textbooks on the classes you have been assigned in MiSiS as of that date.

After you book your appointment, make sure you read and review the District required forms (click here for forms). You will have to go to Google Form to acknowledge that you have read all the required District forms that must be reviewed. There will also be four (4) forms attached below that you must bring with you to the appointment in order to expedite the distribution process. If you do not have a printer, we will have hard copies but that will require your parent to come to the appointment with you. 

Please make sure you bring your student ID (high school or middle school issued), wear a face cloth covering, and adhere to all social distancing requirements on the date of the appointment.  Those who did not pick up items from their lockers will have one last opportunity to do so during the appointment.
